Texas Tops the Nation for STI Rates—Here’s What’s Happening
When it comes to sexually transmitted infections (STIs), Texas is making headlines for all the wrong reasons. Despite national efforts to curb the spread, the Lone Star State continues to report some of the highest STI rates in the country, with certain counties standing out as hotspots.
The Numbers: Where Texas Stands
· Texas ranks among the top states for new STI cases in 2025.
· In 2023, Texas reported 930 cases of congenital syphilis—that’s 238.6 cases per 100,000 live births, ranking 5th nationally.
· Dallas County: 1,314.5 STI cases per 100,000 residents (7th highest among large U.S. counties)
· Travis County: 1,160.3 per 100,000 (14th)
· Harris County: 1,041.4 per 100,000 (17th)
Which STIs Are on the Rise?
Syphilis: While some STIs are declining, syphilis—especially congenital syphilis—continues to climb. The number of congenital syphilis cases in Texas is a major public health concern, with 279 stillbirths and infant deaths reported nationally.
Chlamydia: Remains the most common STI, with over 1.6 million cases reported nationally in 2023.
Gonorrhea: Nationally, cases dropped by 7.7%, but Texas counties still report high rates.
Why Are Rates So High?
Several factors contribute to Texas’s high STI rates:
· Limited access to affordable healthcare and regular testing in some areas
· Inconsistent sex education across the state
· Stigma and misinformation that discourage people from seeking testing and treatment
· Disparities affecting Black and Latino communities
